at the Lake, Inc. would like to welcome you to our agency. We are
here to assist you with your home care needs by attempting in every way
possible to provide you and your family with a caregiver to assist in the
care of you or your loved one.
Our team of
conscientious and compassionate caregivers work hand in hand with our
four owners who operate the office and have over 39 years of experience in
the field of home health and home care.

Lisa M. Smallwood
Financial Director/Human
Resource Manager |
Lisa has an Associates Degree in Health Care Technology
Management, Certificate of Long Term Health
Care, Training in Marketing Skills, HIPAA compliance and OSHA record
keeping and regulation requirements and is a Certified Nursing Assistant.
Her past experience includes Insurance Claims Adjuster, Human Resource
Director, Accounts Manager , Medicaid Billing Specialist and Accounts
Receivables. Lisa handles all our our billing and marketing. |

Karen H. Smith
Senior Home Care Coordinator |
Karen has an Associates Degree in Accounting and
Certificates in Medical Terminology and in ICD-9 and CPT coding. She
manages the home care employees by hiring, scheduling and
supervising the caregivers. She also tracks the day to day
operations of Home Care at the Lake and is our purchasing agent.
Karen is also responsible for communication with clients and their
families. |

Kim O'Neal
Director of Nursing |
Kim has an Associate's Degree in Nursing and has been a
nurse since 1992 with the greater part of her career in the home care
field. She has worked in many areas of Long Term Care, Home Care and
in Hospital settings and specializes as an RN in Elderly Care. She oversees all the healthcare aspects of our company.
Kim is in charge of referrals, reviewing clients charts, supervisory
visits, skill training, new employee orientations, evaluations and quarterly reviews of all our
caregivers. As the Director of Nursing, Kim implements and ensures
that policies and procedures are in place and followed. She also handles
complaints and makes sure they are thoroughly investigated. |

Bette Isereau
Nursing Field Supervisor/ Assistant Director of Nursing |
Bette has a Masters Degree in Education and an Associates
Degree in Nursing with licenses in both North Carolina and New York.
She speaks French fluently and also some conversational Spanish. Her
nursing experience includes staff support, hiring and training in rehab,
home health, private duty, public health, group homes, nursing homes and
community health organizations. Her background also include
providing care to chronic and terminal patients with diabetes, heart and
respiratory problems and wound care. Bette is responsible for giving
presentations, in-service training, account review and management,
implementation of policies and procedures, and ensuring client needs are
met. |

Chonda Trimnal
Administrative Assistant |
Chonda is currently working on obtaining an Associates
degree in Nursing. Chonda is a Certified Nursing Assistant. Chonda is in
charge of orientations. She also obtains necessary information for the
hiring process including, but not limited to reference checks and
scheduling orientations. She also completes all documentation packets for
client case openings, applications, and in-services.

Sharon Carpenter
Assistant Home Care Coordinator |
Sharon attended Montreat-Anderson College and is a
Certified Nursing Assistant. Sharon has over 15 years of work experience
in the home care industry and Department of Social Services. Sharon
schedules and manages employees and their clients needs. She is
responsible for discipline of the employees working under her supervision.
She delegates client complaints to our agency director. She files client
information in their charts.

Karen Coggins
Field Nurse |
Karen has been a Registered Nurse for over 30 years, eight
years of which has been in home care. Karen directly supervises employees
in their client’s homes making sure they are demonstrating necessary
skills and working on a professional level. Karen also assesses clients
post hospital stay and meets with potential clients and their family
members regarding new case openings for clients. Per company policy, any
client complaints are immediately referred to the office.
